在門診室，35歲的孫女士向我敘述了一大堆的身體不適：頭暈、失眠、健忘、胸悶、背痛、肢體麻木?? 掀開衣服，在孫女士的背部，數個紫黑色圓形痕跡交錯重疊，顯然是拔罐所致。孫女士告訴我，她曾經輾轉就診於內科、外科、中醫等科室，做了很多檢查，均未發現身體異常，服藥、理療等也不見好轉，醫生擔心她將來會發展爲精神疾病，才介紹她來看精神科醫生。

實際上，孫女士患的是一種以身體不適爲主要表現的心理疾病，也被稱爲軀體形式障礙。進一步諮詢瞭解到，孫女士近一年來情緒非常糟糕，常常和同事吵架。她甚至討厭4歲的兒子，嫌兒子調皮，說話太多，往往因此怒氣衝衝而打罵兒子。這些不良情緒提示，孫女士存在某些心理壓力。一方面，這些壓力被轉化爲不良情緒發泄到了同事和兒子身上；另一方面，她又壓抑自己，壓力不能充分釋放，身體則以不適的症狀形式表現出來。

在我的繼續引導下，孫女士終於鼓起勇氣，直面內心痛苦，說出了平時不願啓齒的心結（壓力根源）。

10年前，孫女士遇到了現在的丈夫。當時的他一表人才，爲人仗義，凡事敢想敢做，很快贏得了孫女士的芳心。兩人情投意合，山盟海誓，私訂終身。可是，孫女士的父母堅決不同意這門婚事，她因此和父母吵翻了臉，最終和他結了婚。經歷了這些磨難，她向父母表示，一定要過出個模樣來，讓大家看得起。

到現在，夫妻倆做了幾樁生意，不僅沒掙着錢，還欠了一屁股債。兩人均沒有固定工作，基本的生活支出都要靠別人照顧。而且，丈夫花錢大方，常常和朋友們一塊吃喝，往往酒醉纔回家。孫女士惦記着家庭困境，嫌丈夫不爭氣，卻害怕跟父母說出來，因爲當初的選擇是自己的決定。孫女士的內心越來越自卑，在別人面前卻表現得越來越要強。於是，心理壓力越來越大，最終演化爲身體不適。

在對孫女士進行治療時，重點是緩解她的心理壓力。首先，調整認知。讓孫女士認識到自己的身體不適是心理壓力所致，而非真正的軀體疾病，以減輕其過分的擔心和關注。同時，告訴孫女士接受自己的心理和家庭困境，與丈夫多溝通，共同商量解決的方法，避免相互指責。
